Referring a patient or client

For discharge planners, case managers, physicians, and attorneys. Send us the basics and our intake team will confirm coverage, payer fit, and start date, usually the same business day.

Intake turnaroundSame-business-day response for referrals received before 3pm.
We acceptPrivate pay, long-term care insurance, workers' compensation, and case-managed care.
